Most gfx cards now have two ports on the back, to answer your question
you can do it either way. A while ago I ran triple head using two
matrox cards from IBM pSeries boxes. Those two cards didn't have VGA
BIOS chips in them which meant a few extra options (thanks Dave!) but
were better than the onboard S3Savage which had to be run in
framebuffer mode which was very CPU hungry.
Also lookup "Xinerama" (and Xrandr these days).
