According to this:

Sense 0x04 is a "Logical unit not ready" error - there's another byte
which might be
around somewhere if you turn up the logging which might suggest why,
but it probably won't be that useful.

The controller is timing out the drive after issuing a Read(10)
command. The probably cause is the drive being unable to read the
disc. If it's CD specific, check the CD surface - it's probably
either dirty or scratched. If not, either the lens is dirty (possible
if you've been ripping dusty CDs) or the drive is knackered.

I'm pretty sure it's not connector related
