Thank you all for your suggestions. As it happens, I've taken none of
them...

I looked at RTFM but got stuck on a dependency for RT, probably not a
problem if I didn't use ClarkConnect on my server, but I'm not about to
change that.

I also looked at elog and it looked OK, but didn't offer an easy way to
do user logins. I'm also happier with a MySQL backend.

SATYR looks like a very nice simple application and probably would have
done everything I wanted, but then I found zenTrack and it's very easy
to set up and offers the ability to log requests via email. Once I get
that feature working I'll be set :)
