Do it yourself. Zero additional cost over running your own servers. Use
something like everydns.net to provide geographically-dispersed slaves.

Doesn't matter. The infrastructure doesn't care what records it serves -
it just needs the right zone files...

UK servers seem to cost quite a bit more than US servers. There are
certain reasons for avoiding the US at the moment (e.g. DMCA & its abuse),
but you're probably not worried about that.
