I presume that this is recorded from DVB. In that case, you will
need to fix up any A/V desync with ProjectX:

$ java -jar ProjectX.jar -id $VPID,$APID -out $OUTPUT -name $OUTPUT.m2v $INPUT
$ mplex -f 3 -o $OUTPUT $OUTPUT.m2v $OUTPUT.mp2

then you can feed the resulting $OUTPUT file (which is MPEG2) into
avidemux and cut out the ads.
