Writing Apache Modules with Perl and C is fantastic but dated (1.3 not 2.0),
O'Reilly's "scary eagle" book.

Stas Bekman and Eric Cholet have their Practical mod_perl, this is Apache 2
and the newer mod_perl side. This is well regarded by serious Perl people,
and Eric got a bottle of malt whiskey yesterday from the London.pm for his
YAPC interface for running conferences sites.
