Canon (or at least, mine) has a mixture of both. You can replace just the
cartridges, or you can replace the entire print head assembly. It's not
that much more expensive for a print head + full set of inks, and tends
to be needed every now and again.

Of course, my printer dates from a time when ink cartridges were
significantly cheaper than the printer (i.e. the printer was about
£300 - the cartridges were still about £20).
